How to submit an applicant:

There are 3 options for completing the form:

You have a person in mind, but you don’t have all of their professional information, fill out the form with their contact info and the Awards Committee will follow up
You have a person in mind and you have most of the information to be used for the review
You have been nominated and you would like to submit your information for the review

Some examples include: Advanced certifications, continuing education, publications, innovative treatment approaches used. List examples of service to the profession. Some examples include: APTA Membership, involvement in APTA activities, serving as a clinical instructor for PT and PTA students, guest lecturer in PT education, advocacy for the profession at local, state or national levels. List examples of service to the community. Some examples include: volunteer work, pro-bono work, community health screenings, community health lectures, other work to improve the health needs of society.
